Job summary
We are looking for a friendly, enthusiastic and motivated person who enjoys working as part of a team. It is an opportunity to join the Integrated Care Team as the Integrated Care Team Coordinator. You must have excellent communication and IT skills commensurate with this role.

The position is based at Gosport War Memorial Hospital for Gosport 1 Integrated Care Team and including when necessary cross cover for Gosport 2.

ICT's which will be provided in line with service needs.


The work delegated to you will always be within your competency levels, you will receive both formal and informal training.

There is peer support available with other staff members.

The post holder will proactively and positively contribute to the successful overall performance of the Trust.

We are looking for people who will deliver excellent levels of customer service to all patients/visitors and staff at the Trust.


The post holder needs to be innovative and organised developing effective ways of working to create strong partnerships and relationships with all stakeholders to support the Team Manager and develop an organisational culture that fosters collaborative working among all staff groups, to ensure a focused commitment to delivering quality services and outcomes.

Main duties of the job


To provide day to day coordination of Integrated Care Team activities ensuring the ICT is monitoring, reviewing and where necessary proactively managing patients on the caseload to prevent admission to hospital where clinically safe to do so.


To ensure patients with long term conditions are being appropriately managed as part of ICT core services and that any exacerbations of conditions are picked up at the earliest opportunity.

In co-ordination with primary care coordinate the deployment of both health & social care resources in support of local admission avoidance strategies

Qualifications

Essential

  • 5 GCSE/ 'O' levels including English & Maths Grade C.
  • NVQ Level 3 in Business Administration / Vocational qualification e.g. RSA III,
  • OR
  • 3 years plus experience in an senior administrative/ supervisory role

Part-time hours: 30 per week

£25,147.00-£27,596.00 per year
