Description
Join the newly formed Community Based Care Team as a Transformation Manager and play a pivotal role in delivering continuous improvements in health and wellbeing services across community services and Primary Care.
You will work across South East London with Place teams and Providers to lead transformation projects , supporting prevention and integration efforts, working closely with Primary Care leads and wider community partners.
Your responsibilities will include facilitating collaborative working, developing integrated neighbourhood initiatives and providing leadership for a range of transformation programmeso Managing the commissioner network
Primary, Secondary Care and Community interface
o Development and implementation of practical improvement actions around four key areas
o Development of approach to monitoring relationship using data
o Feeding into Chief Medical Officer relationship-led work
Community Commissioning
o Data returns / reporting / improvement projects
o Supporting commissioner/contract rounds
o Supporting a commissioner network.
Our partnership brings together six local authorities, over 200 general practices (operating within 35 Primary Care Networks), Guy's and St Thomas' Hospital NHS FT, King's College Hospital NHS FT, Lewisham and Greenwich NHS Trust, South London and the Maudsley Mental Health FT and Oxleas FT.
Importantly, the ICS seeks to be connected to the communities we serve (circa 1.92m residents) and work with the widest possible range of community, voluntary and third sector groups and organisations in each borough.

Educated to Masters level or equivalent level of experience of working at a senior level in specialist areaEvidence of continued professional development
Demonstrated experience of co- ordinating projects in complex and challenging environments.
Experience of managing risks and reporting.
Experience of drafting briefing papers and correspondence for a senior audience.
Experience of monitoring budgets and business planning processes.
Knowledge and experience of specialist field (BCYP)
Interpreting national policy for implementation
Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service delivery
Ability to make decisions appropriately and plan over short, medium and long -term timeframes and adjust plans and resource requirements accordingly
Ability to Provide and receive highly complex, sensitive and contentious information and present complex and sensitive information to large groups and senior stakeholders
Numerate and able to understand complex financial issues combined with deep analytical skills.
Knowledge of Financial Systems e.g. monitoring budget management, processing invoices and procurement
Comprehensive knowledge of project principles, techniques and tools, such as PRINCE2
Ability to provide and receive complex, sensitive and contentious information and present complex and sensitive information to large groups and senior stakeholders.
