    Liquid Personnel is currently recruiting experienced and Qualified Mental Health Practitioner to join a busy Mental Team based in West London.

    To be successful in this role you must have a relevant Professional registration (SWE, NMC OR HCPC), be eligible to work in the UK, hold a qualification in Social Work, Nursing or Occupational Therapy at degree level, and have post-qualified experience.

    Your main responsibilities as aMental Health Practitioner:

    • Maintain a personal awareness of legislation, policies and procedures, particularly those relating to the Care Act, The Mental Health Act, the Mental Capacity Act, safeguarding and self-directed support in order to apply a strong evidence base to ensure consistency and a high quality of service delivery.
    • Provide high quality assessment and intervention to service users experiencing a wide range of mental health problems, promoting recovery and independence to identify appropriate pathways and treatment plans with service users.
    • Care planning.
    • Care Coordination
    • Managing a caseload in accordance with the Care Programmed Approach (CPA)
