The Humber NHS Teaching Trust is looking to recruit an Apprentice Assistant Practitioner within our new Virtual Ward.

As a Trust, we see the value in supporting the development and progression of our people through apprenticeship pathways, combining vocational learning with on-the-job experience.


You will undertake a Level 4 Healthcare Support Practitioner Apprenticeship whilst you work closely with your new team to gain the essential skills, knowledge and behaviours for this new role.

The duration of this apprenticeship will be 15-18 months and on successful completion you will progress into a Band 4 Assistant Practitioner role.


Would you like to be part of our new virtual ward, as part of the Ageing Well Programme, including integrated service delivery, enhanced health in care homes, and anticipatory care targeted at people living with frailty to help them stay independent and healthy for as long as possible? Assistant Practitioners will work in our Virtual Ward, providing clinical care in patients' own homes and Care Homes.


Our new service will be delivered across Whitby, Scarborough, and Ryedale Community, encompassing travel across market towns, the moors, the seaside and the coast within daily working.

Working alongside the therapy team and community nursing team undertaking patient home visits to facilitate discharges from hospital, prevent admissions to hospital, carry out assessment & rehabilitation and clinical nursing duties


This could include:

  • Falls assessments
  • Assessment for assistive devices for independence and safety at home
  • Phlebotomy
  • Carrying out ECGs
  • Pressure area care
  • Wound care
  • Management of the deteriorating patient and appropriate escalation
  • Palliative care
