    Job summary

    EAST:MK Primary Care Network has exciting opportunity for an advanced nurse practitioner to join our team. We are looking for someone to join us on a full-time basis for hours per week, over 5 days between Monday and Saturday, on a rota basis across the 4 surgeries that make up the PCN.

    Main duties of the job

  • Acting as an expert practitioner, demonstrating advanced clinical competence & knowledge base beyond those associated with conventional nursing roles.
  • Working autonomously, providing high quality and timely patient centred care.
  • Be an independent prescriber, working within the current guidelines, reviewing, & monitoring patient medications in accordance with the formulary.
  • Having experience of delivering face to face/telephone/e-consult/video consultations as well as home visits.
  • Directing care appropriately and managing complete episodes of care independently.
  • Undertaking consultations with patients of all ages including children as an autonomous practitioner & using own clinical judgement, diagnosing, treating, referring and/or discharging patients.
  • Working across the MDT teams, signposting/drawing upon expertise of all members, supporting & meeting patients' needs and best interests.
  • Recording clear and contemporaneous consultation notes to agreed standards, ensuring accurate read coding.
  • Participating in, and contributing to, all clinical targets (QOF, QIS, ES etc).
  • Recommending, instigating, & explaining appropriate diagnostic tests, investigations, and interpreting findings and reports.

    Person Specification

    Qualifications

    Essential

  • A registered nurse with the Nursing and Midwifery Council (NMC).
  • A Masters degree from the centre for Advancing Practice accredited MSc advanced practice programme or completed the centres e-portfolio (supported) route.
  • A qualified independent nurse prescriber, on the NMC register.
  • Meets NMC revalidation requirements & standards for registered ANP working at advanced level.
  • Experience

    Essential

  • The ability to record accurate clinical notes.
  • Experience of working in a management role and evidence of leadership skills.
  • Experience of working in a wide range of primary care settings to include clinical triage experience.
  • Ability to problem solve.
  • Knowledge of health promotion strategies.
  • Understand the requirement for PGDs and associated policy.
  • Polite and confident, flexible and cooperative.
  • Motivated & forward-thinker.
  • Problem solver, with the ability to process information accurately and effectively, interpreting data as required.
  • High levels of integrity and loyalty.
  • Sensitive & empathetic in distressing situations.
  • Ability to work under pressure in stressful situations.
  • UK driving licence, with exclusive access to a vehicle.
