    Job Summary

    Main Duties of the Job

    The post holder will have a crucial role in the CHC service, operating within the National Framework for Continuing Healthcare to provide an effective, efficient, and fair service to the people of Sussex.

    • Delivery of various functions within the CHC service, including assessments, reviews, care planning, and case management.
    • Opportunity to engage in additional CHC areas like audit, appeals, retrospective assessments, and more.
    • Support and development to work efficiently and confidently across the whole pathway.

    Job Responsibilities

    • Supporting and mentoring staff for continuous improvement in the CHC Team.
    • Managing a patient caseload and contributing to performance enhancement.
    • Ensuring quality assurance in care delivery.
    • Undertaking statutory reviews to meet client needs and care pathways.
    • Facilitating CHC assessments and reviews while providing expert support to professionals and patients.
    • Participating in the verification process and managing fast track referrals.
    • Completing retrospective CHC assessments and ensuring stream plans are effectively executed.
    • Supporting staff training for role fulfillment and maintaining compliance with NHS CHC guidelines.

    Travel may be required occasionally for assessments and reviews, necessitating access to transportation.

    The job description and person specification are subject to ongoing review to meet departmental and organizational needs. Flexibility is expected from all CHC staff to serve the needs of the service.

    Qualifications

    Essential

    • Educated to degree level in Nursing or equivalent field with current registration with the relevant regulatory body.
    • Evidence of Continued Professional Development.

    Experience

    Essential

    • Prior experience in a health or care setting.
    • Comprehensive understanding of health and social care environments and respective roles.

    Desirable

    • Skills in negotiation, conflict management, and operational processes.
    • Ability to address complex issues effectively and work with safeguarding policies.

    Skills and Knowledge

    Essential

    • Proficiency in patient assessment, care planning, and decision-making.
    • Capacity to manage workload efficiently and handle sensitive issues both verbally and in writing.
    • Knowledge and experience in Safeguarding and the Mental Capacity Act.
    • IT proficiency with Microsoft Office applications and intermediate keyboard skills.

    Desirable

    • Understanding of national and local policies related to NHS Continuing Healthcare.
    • Awareness of Freedom of Information and Subject Access Requests processes.
