    Job summary

    The Clinical Case Coordinator role sits under the Surrey Heartlands Continuing Health Care Team and works closely and collaboratively within the immediate team and external agencies, building working relationships with all including the patient and their families and carers. Primarily the role involves the assessing of individuals for CHC eligibility, writing of a robust and comprehensive assessment, liaison with multi disciplinary agencies and the gaining of all relevant information and evidence in support of the final recommendation.

    The case coordinator will have access to support and training in all aspects of CHC in order to improve their knowledge and skills.

    Main duties of the job

    Once a patient is assessed as being eligible for NHS Continuing Healthcare (CHC). Our team commissions a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs.

    You will be responsible for the assessment of Continuing Healthcare patients.

    You will use your existing and new knowledge and clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS Continuing Health Care and NHS Funded Nursing Care.

    You will work closely with the team, hospitals, community services and care homes to maintain and improve systems and processes to facilitate a smooth referral, assessment and review process to support patients.

    Surrey Heartlands ICB is responsible for the decision making and commissioning processes, and work collaboratively closely with the Local Authorities.

    Agile working is what we offer. However, we have several offices throughout Surrey Heartlands ICB, which can accommodate working from the office if required. Ideally you will live within the Surrey Heartlands geographical area, as you will be expected to travel across Surrey for assessments and travel to your induction and meetings as required by the service. Our main Headquarters are in Epsom and Woking. Considering a mile radius would be advisable. Please consider this before applying.
