AHP Frailty at the Front DoorWe are delighted to share this exciting new opportunity for Physiotherapy staff to join us to continue to develop our AHP front door frailty service within Raigmore Hospital, Inverness.
Fully supported by a team of Advance Practitioner AHPs, these posts will work across the interface between acute and community settings to support those patients identified with frailty to maximise their independence and wellbeing within their local communities,
deliver early intervention to support timely discharge and to prevent avoidable hospital admission.
