Description
An exciting opportunity for Community Nurses has arisen both in the Newmarket and Mildenhall and Brandon Integrated Neighbourhood Teams. The other INT localities are based in Bury Rural, Bury Town, Haverhill and Sudbury.The INT accommodation 'Hubs' are the base for a wide range of other health professionals and agencies including Adult Social Care, social prescribers, GPs, police and mental health team members so we are able to work together in a fully integrated way to ensure the best care is delivered to the patients within our localities.
The successful applicant/s will join a dynamic, friendly team of community nurses, community matrons, therapists, social prescribers, social care staff, INT coordinators and local administrators, supported fully by a locality Team Manager, and Localities Lead.
You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi-disciplinary team and across the West Suffolk Alliance.
You will be assessing the patients by carrying out home visits and planning and implementing a patient's individualised care plan.
Within the INT localities you have access to a range of treatment facilities and may be involved in delivering nursing care in specialist clinics, such as the leg ulcer and doppler clinics.
You will be using a wide range of community nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting.

The West Suffolk Hospital in Bury St Edmunds provides acute and secondary care services (emergency department, maternity and neonatal services, day surgery unit, eye treatment centre, Macmillan unit and children's ward).
Adult and paediatric community services, provided in collaboration with West Suffolk Alliance partners, include a range of nursing, therapy, specialist, and ongoing temporary care and rehabilitation, some at our Newmarket Community Hospital.

To contribute to the delivery of high standards of health care to patients within their own homes, residential care homes, and in clinics, by assessing, planning, implementing and evaluating care.
You may be required to work a 24 hour shift pattern in the future, should this service be commissioned by the Trust.
To assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
To work within the integrated team to prevent unnecessary admission to hospital.
To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
~ Planning and organisational Skills:
Be accountable for own professional actions as determined by the Nursing and Midwifery Council code.
Comment on draft policies.
Ensure adherence to Trust policies, and procedures.
Take part in clinical supervision as per Trust Policy.
Provide mentorship for students undertaking pre/post registration course.
Complete risk assessments and incident forms as required.
~ Triage, assess, plan, implement and evaluate all aspects of patient care, and develop care plans which may need to be delivered from a range of options.
Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required.
Use tactful and persuasive skills to gain the patients co-operation in their care management plan.
Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated.
Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g.Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
