Bedfordshire Hospitals NHS Foundation Trust has an exciting opportunity for an exceptional, dynamic Discharge Planning Officer to join the Integrated Discharge Planning Team based at Bedford Hospital, with occasional rotation between Bedford Hospital North Wing.


Working within the Integrated Discharge Team (IDT) you will be part of a team providing support to patients, relatives and staff through admission to discharge.


The post holder is the single point of contact in relation to discharge for members of system wide multi-disciplinary teams, patients and their families / carers therefore it is essential that the post holder maintains an excellent working relationship with all stakeholders.

The post holder will have an understanding of partnership services available along with criteria for referral under the National '

Hospital Discharge Service:

Policy and Operating Model' and work in line with the Mental Capacity Act 2005, and Care Act 2020 ensuring rights are maintained.


You will be a compassionate, enthusiastic and organised person with excellent communication skills who has the ability to work autonomously along with working well under pressure.

You will work closely with and under the direction of the Senior Team.

  • To work as part of a multidisciplinary team in order to facilitate safe and timely patient discharge from Bedford Hospital.
  • The post holder will have the responsibility to ensure that all discharges that come under the Integrated Discharge Team framework from his/her wards are safe and expedited within a reasonable time frame.
  • The post holder is the single point of contact in relation to discharge for members of system wide multidisciplinary teams, patients and their families / carers therefore it is essential that the post holder maintains an excellent working relationship with all stakeholders.
  • To ensure that patients are communicated with on a regular basis and to engage with families when required and prior to discharge.
  • To have an understanding of partnership services available along with criteria for referral.
  • Work within the Mental Capacity Act 2005, ensuring rights are maintained.
  • Ensure good working relationships are maintained with all members of the multidisciplinary team.
  • Attend daily Multidisciplinary Team Meetings on wards to facilitate effective Complex and Non-Complex Discharge Planning.
  • Work on the wards daily to assess and plan discharge requirements.
  • Manage own caseload under the direction of the Senior Discharge Team.
  • To keep documentation clear, accurate and up to date; to include both paper and electronic systems.
  • Ensure that patients with complex discharge needs are identified on admission, working with partners to ensure accurate and timely referrals are made when required.
  • In collaboration with the ward staff identify situations that may cause delays in discharge and escalate these to the Senior Discharge Team.
  • Provide daily updates to Senior Discharge Team.
  • Promote and demonstrate a flexible approach to working hours to ensure that families are met outside of normal working hours if required. Rotational weekend working is in place to ensure that we have a 7 day service.
  • Rotate to other areas that come under the Integrated Discharge Team as needed and directed by the Senior Discharge Team.
