- To work independently with CYP, who are under the care of the LSCIC consultants on the ward or therapy environment, to deliver and evaluate their care and report back to registered nursing staff as appropriate.
- To support non SCIC CYP with basic nursing and personal care.
- To deliver and evaluate care and assistance to CYP both in the hospital and community settings, home, nursing homes, NHS facilities.

- To undertake risk assessments on CYP prior to leaving the clinical area with appropriate members of the MDT
- Provide an appropriate handover of their needs through written and verbal education and practical demonstration to CYP, families, and community professionals in a variety of clinical/community settings.
- Provide a written and verbal handover of outcomes of outings/meetings or discharge as required by Case Managers /MDT.
- Ensure that when going off Trust grounds with CYP any medication, supplies and equipment are available as required.
- Undertake appropriate risk assessments prior to commencing and during community activities.
- To report any untoward incidents to the Ward Manager/Case Manager and participate in the Trust Risk Management process as appropriate.
- To report to the appropriate member of staff any problems encountered on activities whilst respecting confidentiality.

- To undertake a familiarization session relating to the SCIC vehicles in order to become a regular driver of the SCIC vehicles. Job summary
The main purpose of the role is to support the care, rehabilitation, reintegration and discharge of Children and Young People (CYP) with spinal cord injury and those with associated complex needs. Post holder may be required to support with the care of other CYP on Coxen to ensure quality and safe clinical care for the service and when this is deemed appropriate. In conjunction with other members of the Multi-Disciplinary team. Promote and encourage social reintegration for CYP with spinal cord injury through sports, leisure and activities of daily living. Ensure continuity of appropriate care education and discharge preparation through close liaison and handover of care needs to CYP, families, and relevant community professionals.
