    Job summary

    The primary role of this post is to assist the PCN Clinical Director in developing and delivering Primary Care Network (PCN) pharmacy led services, providing pro-active leadership on medicines optimisation and prescribing across the PCN multidisciplinary teams to the benefit of our patients and their carers, maximising cost effectiveness and minimising risk.

    Main duties of the job

    You will be responsible for the delivery of all medicine related contractual requirements and local incentive schemes, QIPP and other quality or cost effectiveness initiatives. The post holder will advise on medicines supply and medicines optimisation across the PCN while also providing patient facing clinical services as part of the PCN Pharmacy team. The post holder will provide leadership to and management of the PCN Pharmacy team and the pharmacy led services within the T4H PCN GP Practices and link in with key stakeholders to improve patient experience, service provision and affect positive change. The post holder will develop, implement and over-see a PCN wide medicines management strategy, to support a consistent approach across all practices within the PCN and provideconsistent patient care.The post holder will continuously seek to improve the quality and effectiveness of prescribing through governance audit, clinical audit and education to improve performance against NICE standards, prescribing guidance and other clinical and professional standards

    Qualifications

    Essential

  • Masters degree in Pharmacy (MPharm) or equivalent plus 12 month preregistration experience
  • Membership of the General Pharmaceutical Council (GPhC)
  • Clinical diploma or relevant experience
  • Post-Graduate Independent Prescribing Qualification or working towards

    • Experience

    Essential

  • Commitment and evidence of formal continuous professional
  • development (CPD)
  • Minimum of 5 years experience as a clinical pharmacist and preferably at
  • least 10 years post registration.
  • Experience of undertaking evidence based reviews
  • Evidence of experience in using independent prescribing
  • Experience of a patient focused role and consultation skills to apply
  • knowledge in a format patients/carers can understand
  • Experience of providing prescribing and medicines management advice
  • Experience of providing prescribing advice to GPs
  • Experience of running a patient clinic
  • Previous experience in delivery of training and education
  • Experience in leading staff and clinical supervision
