About the Role:

The Colte Partnership is looking to recruit an experienced care co-ordinator to join our growing executive team.

You will primarily manage our extended access service, providing patients with the best possible care and service at the best time for them.

This role is evening and weekend based.

The hours per week will be as follows:
Wednesday and Thursdays - 6pm to 8pm

2 Saturdays per month - 9 to 5pm (3 Saturdays on a 5 week month)


You will be based at The Colne Medical Centre, Walton Medical Centre and, once a month, at The Tiptree Medical Centre.


This is a new post developed to deliver better outcomes for vulnerable people and/or those living with frailty, with a key focus on supporting patients and health care providers.


As a care co-ordinator, you will be involved in proactive, person-centred care planning, overseeing and coordinating patients' care when needed, by bringing together the different specialists whose help the individual may need.

About Us:

The Colte Partnership is a GP super-partnership, formed in June 2017. It brought together 9 local GP surgeries across Colchester and Tendring, serving approximately 98,000 patients.

With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.


As well as general practice, we offer extended access appointments to enable all of our patients to access medical care at evening and weekends; we work collaboratively with external agencies to provide in home services and social prescribing; we are working to be at the forefront of clinical research; and have many more exciting ventures on the horizon.

We also work as three collaborative primary care networks (PCNs) which span across Colchester and Tendring:

  • Ambrose Avenue Group Practice, Riverside Medical Centre & Ardleigh Surgery
  • Mersea Island Medical Practice, Tiptree Medical Centre & Rowhedge Medical Centre
  • Walton Medical Centre, Colne Medical Centre & Wivenhoe Medical Centre


There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.

You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.
