Senior CHC Nurse Assessor Band 8b (Hybrid)
**£315 per day (Inside IR35)
3 Month Contract**
Birmingham, West Midlands (commutable from Solihull, Coventry, Redditch, Walsall, Wolverhampton, Worcester, Tamworth)

My Client are an NHS Trust based in West Midlands and are seeking a Senior CHC Nurse Assessor on an initial 3 month contract basis (37.5 Hours) until 31/03/2023 Paying £315 per day (Inside IR35).

Job Summary

The post holder will undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs including the assessment, review and monitoring of care for those whose health needsare funded via NHS Continuing Healthcare within the dedicated time scales.

This will include retrospective and appeal cases.

The post holder will act as a Case Manager for individual clients within Birmingham Solihull CCG who are receiving continuing healthcare funding to assess the needs of the client and their family to facilitate, plan and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environment.

Experience
Essential

Previous experience of undertaking Continuing Care Assessments
Significant experience of working patients with complex and longterm health needs, disabilities and those with life limited/palliative or end of life
Experience of negotiating and liaising with multidisciplinary team.
Evidence of undertaking audit and implementing outcomes in a range of settings
Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
Commitment to partnership working across education, health, and local authority settings
Negotiation and conflict management skills and the ability to influence decisions.
Evidence of leadership, management and team development
Evidence of professional/clinical and managerial expertise

Desirable

Understanding of managing a budget and financial processes.
Evidence of having undertaken leadership role in working across boundaries

Qualifications
Essential

Educated to degree level or equivalent level of experience working in a specialist area
Registered Nurse with a current NMC registration or equivalent

Desirable

Other relevant specialist post registration qualification, experience
