    Job summary

    This is an exciting new opportunity to join an established, well supported Clinical Pharmacy Team in Primary Care. The role is pivotal to improving the quality of care and operational efficiency within general practice and PCNs. You will work aspart of a multi-disciplinary team in a patient-facing role takingresponsibility for undertaking structured medication reviews to proactively manage patients with complex polypharmacy and ensure medicine optimisation. You will provide primary prescription and medication support to general practice staff, help support the repeat prescription system, deal with acute prescription requests and support medicines reconciliation on transfer of care. The post holder will also implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices.

    Main duties of the job

    Primary Duties and Areas of Responsibility. Structured Medication Reviews: Consult patients with single or multiple medical problems/long-term conditions to ensure medicine optimisation.Implement safe systems with regards to Shared Care protocols. Advise patients and staff re: common/minor/self-limiting ailments. Provide telephone advice for patients with concerns about theirmedicines.Review medicines usage commonly associated with unplanned / highintensity hospital attendance .Review medicines at discharge from hospital. Support the production and implementation of a repeatprescribing protocol. Identify cohorts of patients at high risk of harm from medicines.Provide advice on treatment pathways and patient informationleaflets.Undertake and implement changes resulting from clinical audits. Medicines safety - You will support the implementation of changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

    Job responsibilities

    Clinical Pharmacist

    The post holder will be anexperienced pharmacist acting within professional boundaries, workingalongside a team of pharmacists in a group of general practices. They will workas part of a multi-disciplinary team in a patient-facing role, undertakingclinical medication reviews to proactively manage patients with long term conditions, complexpolypharmacy ensuring medicine optimisation.

    The post holder will provideprimary prescription and medication support to staff, support the repeat prescription system, deal with acute prescription requestsand support medicines reconciliation on transfer of care. The post holder willalso implement systems for safer prescribing, providing expertise in clinicalmedicines advice.

    The post holder will provideclinical leadership on medicines optimisation and quality improvement andmanage some aspects of the quality and outcomes framework and enhancedservices.

    The post holder will ensure thatthe practice integrates with community and hospital pharmacy to improve patient outcomes, ensure better access to healthcare andhelp manage practices workload. The role is pivotal to improving the qualityof care and operational efficiencies within the PCN.

    Primary Duties and Key Areas ofResponsibility:

    Work closelywith other pharmacy professionals across PCNs and the wider health and socialcare system

    Undertakestructured medication reviews ensuring medicineoptimisation and improved patient outcomes. Patient groups include: frequenthospital attenders, unplanned hospital admissions, care home residents, and thoseon multiple medicines etc.

    Be a prescriber, or be willing toundertake training to become a prescriber, and work alongside the generalpractice team

    Advise patients re:common/minor/self-limiting ailments -while working within your scope of practice, directing patients to self-care in line with nationalguidance. You will signpost patients to the community pharmacy and refer to GPsor other healthcare professionals when appropriate.

    Medicines information - Provide advicefor patients with questions, queries and concerns about their medicines.

    Medicines reconciliation - You will reconcilemedicines following discharge from hospitals and intermediate care, and intocare homes. This includes identifying and rectifying unexplained changes, andworking with patients and community pharmacists to ensure that patients receivethe medicines they need post-discharge.

    Repeat prescribing - You will supportthe production and implementation of a repeat prescribing policy and undertakethe repeat-prescribing reauthorisation process through reviews on patientsrepeat prescription requests, medicines reaching review dates, and throughflagging those needing a review. You will also ensure patients have appropriatemonitoring tests in place when required.

    Risk stratification - You will identifycohorts of patients at high risk of harm from medicines through pre-preparedpractice computer searches. These may include patient, and medicine relatedrisks.

    Service development - You willcontribute pharmaceutical advice for the development and implementation of newservices that have medicinal components (eg: advice on treatment pathways andpatient information leaflets).

    Information management - Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making in General Practices.

    Medicines quality improvement - Youwill undertake clinical audits of prescribing in areas directed by GPs. Youwill feedback the results and implement changes in conjunction with thepractice team and the Senior Clinical Pharmacist.

    Medicines safety - You will support theimplementation of changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.

    Shared Care - Provide support in theclinical aspects of shared care protocols, clinical research with medicines,liaison with specialist pharmacists (including mental health and reduction ofinappropriate antipsychotic use in people with learning difficulties), liaisonwith community pharmacists and anticoagulation.

    Care Quality Commission - You will workwith the general practice team to ensure compliance with CQC standards wheremedicines are involved.

    Public Health - You will support publichealth campaigns, provide specialist knowledge on the public health programmesavailable to the general public.

    Professional Clinical Network - Be partof a professional clinical network and have access to appropriate clinicalsupervision.
