    Job summary

    We are looking for a motivated Practice Nurse toprovide care for the presenting patient, treatment, chronic disease managementand evaluation of care.

    Working in accordance with the NMC Code of Conduct, thepost-holder will require experience in cervical cytology and baby immunisationsessential. The post will play a crucial role in planning, co-ordinating thework of the nursing team under the guidance of the Lead Nurse.

    Main duties of the job

  • To develop,implement and embed health promotion and wellbeing programmes
  • To implement andevaluate individual treatment plans for chronic disease patients that promotehealth and wellbeing
  • To identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to patients health
  • To maintainaccurate clinical records in conjunction with current legislation
  • To ensure readcodes are used effectively by all members of the nursing team
  • To ensure theappropriate maintenance and use of chronic disease registers
  • To implementvaccination programmes for adults and children
  • To delegateclinical responsibilities appropriately (ensuring safe practice and the task iswithin the scope of practice of the individual)
  • To support theclinical team with all safeguarding matters, in accordance with local andnational policies
  • Provide care forminor injuries, dressings, and higher-level wound care, liaising directly,where appropriate with community and secondary care counterparts, escalating toa GP or referring onto community services , leg ulcer clinic/tissueviability etc.
  • Undertakeopportunistic screening when clinically required , blood pressure, BMI,smoking status, alcohol intake etc.
  • Performvenepuncture and electrocardiography and suture and staple removal
  • Assessingproblems presented opportunistically by patients

    Person Specification

    Experience

    Essential

  • Of working in Primary Care.
  • Minimum of 3 years post-registration experience.
  • Knowledge of evidence-based practice and clinical governance.
  • Evidence of appropriate continuing professional development activity to maintain up-to-date
  • Knowledge and on-going competence.
  • Evidence of working autonomously and as part of a team.
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice.

    • Qualifications

    Essential

  • RGN with current NMC pin (if from a nursing background).
  • Evidence of continuing professional development.
  • Current NMC registration
  • Cervical cytology qualification
