    An exciting opportunity has arisen for a Band 7 nurse to provide a highly specialised service to oncology urology cancer patients throughout their care pathway including the overarching responsibility for the support of patients living with and beyond cancer.

    Main duties of the job

    The post holder will work closely with the urology cancer CNS teams and relevant MDT members maintaining and developing nurse led services for the management of patients throughout their pathway from referral to on-going treatment and living with and beyond cancer.

    Patient needs will be identified through a holistic need assessment and co-creating a Care and Support Plan ensuring appropriate referrals for information and psychological support.

    Job responsibilities

    KEY RESPONSIBILITIES

    • PATIENT CARE, CLINICAL EXPERTISE AND CASE MANAGEMENT

    1.1 Manage a caseload of patients, as appropriate within the speciality, delivering individualised and personalised direct patient care.

    1.2 Provide clinical expertise, specialist advice and support across the whole spectrum of the service.

    1.3 Adopt a range of the Chief Nursing Officer 10 key roles appropriate to client group in order to improve and streamline patient care and patient experience.

    1.4 Demonstrate in-depth specialist knowledge, skills and innovative practice in the specialist area, leading the delivery of appropriate evidence-based care, and advanced practice.

    • Ensure the delivery of the highest quality, clinical effective care, and patient experience, prioritising, and utilising research and in collaboration with colleagues in everyday practice.
    • Provide direction and clinical leadership, working in partnership with the specialist team and multi professional team to enhance standards and best practice of care across the service.
    • Promote and lead the development and delivery of a programme of practice and professional development for staff within the speciality.

    1.5 Support, assist and encourage the multi-disciplinary team in resolving complex patient problems, by the provision of new and innovative models of case management.

    1.6 In collaboration with the management team assist the lead the nurse Practitioner contribution to the service, in the delivery of high standards of patient care.

    1.7 Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation, and evaluation of patient care. This will necessitate high levels of autonomy and advocacy.

    1.8 To support the nursing contribution to and participate in Macmillan Service Reviews and the National Quality Surveillance Programme.

    1.9. Makes sure that people affected by cancer are aware that they are interacting with a Macmillan Professional and know the full range of resources and services available through Macmillan.

    2.0 Actively engages with Macmillan Cancer Support to contribute expertise and experience and supports the Macmillan priorities.

    2.1 Takes personal responsibility for life-long learning and personal development through clinical supervision, appraisal and Knowledge and Skills Framework and actively engages with the learning and development opportunities offered by Macmillan.

    2 PATIENT AND PUBLIC INVOLVEMENT AND EXPERIENCE

    2.1 Promote the philosophy of patient centred care and always establish and maintain a supportive relationship with the patient and their family.

    2.2 Promote patient and public involvement and enhance their experience by developing new initiatives that support patient centred care.

    2.3 Contribute to the Directorate and Service strategies for raising the profile of patient and public involvement and embedding this within own sphere of practice.

    • PROMOTE AND ESTABLISH EFFECTIVE TEAM WORKING AND COMMUNICATION

    3.1 Take overall responsibility for ensuring effective communication between all service providers within the speciality.

    3.2 Provide and establish good positive working relationships with the Heads of Nursing, Sister / Charge Nurse and their teams, Education and Practice Development Lead, Matrons, relevant Clinicians, Service Managers and General Managers.

    3.3 Develop and promote working partnerships between the Trust and academic staff at local Universities involved in the provision of nursing and multi professional programmes of education.

    3.4 Actively participate in local, national, and international networking to maintain and develop expert level of knowledge in the speciality.

    3.5 Ensure that the importance and implications of education, research, and ideas are raised in appropriate forums.

    • EDUCATION, TRAINING AND PRACTICE DEVELOPMENT

    4.1 Assist in the implementation of specialist education and training to all members of the clinical team and nursing staff within the Directorate in collaboration with the Education and Practice Development Lead.

    4.2 Actively encouraging lifelong learning and personal development through appraisal and Knowledge and Skills framework (KSF) development plans.

    4.3 Assist in the development of a patient focused education and information giving strategy within the speciality.

    4.4 Working with the Directorate Education and Practice Development Lead ensure that specialist education and training meet the needs of the service, multi-professional clinical teams, patients and carers.

    4.5 Demonstrate continuous evaluation of practice and make changes where appropriate, within the defined speciality.

    4.6 Use Education forums and Networks proactively sharing good practice and innovation with others.

    4.7 Liaise with other groups and assist with the delivery of multiprofessional and interprofessional learning.

    4.8 Provide specialist education teaching, learning, and mentoring for pre and post registration students when required and actively contribute to the delivery of the Universities curriculum.

    4.9 Personal/ Professional Development

    • To maintain accurate records/documentation of enquiries while respecting the right to patient confidentiality utilising written and electronic methods i.e. iPortal and Somerset Cancer

    Registry. Any matters of a confidential nature including information relating to patients, their treatment and diagnosis, individual staff records must under no circumstances be divulged or made available to any unauthorised person(s).

    • To maintain mandatory training updates as per Trust policy
    • To take every reasonable opportunity to maintain and improve your professional knowledge and competence
    • Complete: Site specific specialist training, Oncology Course, Non-medical prescribing, Advanced examination, SACT course, Teaching Course
    • Masters degree education or working towards a relevant masters degree.
    • It is expected that ALL employees of the Trust will take part in the appraisal process each year with the first appraisal taking place within the first 6 months from appointment
    • CLINICAL GOVERNANCE, AUDIT AND RESEARCH

    5.1 Influence and develop policy making and clinical guidelines internally and externally, necessary to support the specialist nursing service.

    5.2 Lead and participate in audit and research and benchmarking activities to ensure effective safe and evidence-based practice.

    5.3 Promote and encourage nursing practice research in all relevant clinical areas supporting the GEH Non-Medical research strategy.

    5.4 Provide and promote clinical excellence that reflects the Trusts Clinical Governance Strategies to include meeting the Standards for Better Health.

    5.5 Promote the speciality by publishing and presenting innovations and research findings locally and nationally.

    5.6 Actively promote the specialist service through dissemination of specialist knowledge, both locally and nationally, establishing robust networks.

    5.7 Identify and develop proposals and business cases as required for future service innovation.

    5.8 Act as a role model and clinical resource demonstrating high standards of care and providing clinical leadership in the specialist area.

    5.9 Ensure that audit and research findings inform and change practice as appropriate.

    • RISK MANAGEMENT AND PATIENT SAFETY

    6.1 Ensure that specialist knowledge, skills and innovative practice is delivered, to provide a high standard of patient care and maintain patient safety.

    6.2 Actively promote infection prevention and control awareness using best practice to reduce the potential for cross infection.

    6.3 Ensure patient safety through programmes of effective and relevant education and staff compliance with policies and practices.

    6.4 Facilitate the implementation of new ways of working that benefits patient safety and the patient user experience.

    6.5 As a clinical expert provide advice and support to assist with the management of complaints relating to their speciality and implement action plans or change of practices.

    • SPECIALITY SPECIFIC

    8.1 Delivers planned clinical work within the speciality, including Practitioner led clinics & other speciality specific commitments, effectively articulating a clinical perspective on a patients condition with colleagues.

    8.2 Carries out a range of clinical procedures following competence training including:

    • Holistic needs assessment
    • Health and wellbeing clinics
    • Health and wellbeing events
    • Triage of Patient on the patient centred follow up pathway.
    • Coordinates and manages patients of the patient centred follow up pathway.
    • Communication of results, treatment plans, referrals and sign posting to wider MDT
    • Non-Medical Prescribing, Advanced patient examination
    • Venepuncture
    • Cannulation
    • ECG
    • Wound care

    8.3 To deliver a range of services to meet the needs of the patient in their surgical and Oncological pathway, from outpatients to discharge, including patient centred follow up care.

    8.4 The post holder is empowered to make clinical decisions & initiate & carry out interventions based on their own interpretation of the clinical situation.

    8.5 Discharge patients under nurse led discharge & ensure the provision of continuity of care including follow up visits & advice.

    8.6 Undertakes a key co-ordination role within the speciality to ensure cohesion within the multidisciplinary team and patient management.

    8.7 Actively contributes to reconfiguration projects, service redesign developments and the Directorate strategy & business plan as required.

    Person Specification Experience and knowledge

    • oExtensive clinical experience, minimum of 3 years post registration experience to include at least 1 year of management/leadership experience at band 6 Experience or above.
    • oOn-going personal/professional development: Evidence of master's level study; Non-medical Prescribing; Advanced Health Assessment Course Specialist Cancer Nurse Training

    Qualifications

    • oRegistered Nurse on NMC Register Degree in Nursing or health/nursing related studies.
    • oEvidence of working towards specialty related Degree or evidence of continuing academic progression in combination with equivalent Oncology experience
    • oRecognised certified course Cert in teaching or equivalent.
    • oWorking towards a master's degree
    • oResearch qualification.
