An opportunity for a Dietetic Practice Assistant has arisen to join the Nutrition and Dietetic Service at Hertfordshire Community NHS Trust in East and North Hertfordshire.
This is a very rewarding role, helping the staff in care homes develop their knowledge and skills to help their residents.
The post holder will be expected to:
- Deliver a set training programme on the identification of malnutrition risk and the use of food based fortification for its treatment.
- To support the dietitians with completion of audit data onto established software packages.
- To support the dietitians in reviewing patients who have an agreed care plan for the treatment of malnutrition.
- Participate in placement experience for student dietitians.
- Staff are well supported and growth is encouraged through appraisal, shadowing, mentoring and training.
09:30-16:00 or 10:00-16:30.
The post requires a full valid driving licence and access to a car to use regularly for business purpose (unless you have a disability as defined by the Equality Act 2010).
- The role includes care home audits and education in designated care homes, representing Hertfordshire Community NHS Trust. It will include collecting agreed audit information, assessing the gaps identified as part of this audit, and from there delivering tailored education sessions to all the care home staff.
- The post holder will be responsible for liaising with the care homes to agree on when the education will be delivered. This is primarily in a facetoface format, and occasionally in a remote format on a system such as MS Teams.
- This post will work alongside a band 5 ICT dietitian playing a vital role in our Herts wide service, in which we have over 10 staff working at levels band 4 and 5 in multiple specialties.
