    Job summary

    Integrated care is all about providing better care and support to our patients, users and the people of Oldham by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.

    You will join our multidisciplinary Community Rehabilitation and Falls Team carrying out holistic assessments of patient needs and develop, implement and evaluate treatment programmes. There is a varied caseload to support adults and older people within their own homes, including care homes.

    Main duties of the job

    Working with band 7 team lead you will support the delivery of an efficient and effective service.

    You will be an autonomous practitioner and work as a key member of the multidisciplinary team and supervising junior staff.

    You will have an active role in coordinating new referrals, managing waiting lists and supporting the team to maintain patient flow.

    You will have a broad range of experience managing medical, surgical, neurological and mental health conditions and ideally some community experience.

    The service is community-based and would require you to be able to travel to various locations across Oldham on a daily basis.

    Qualifications

    Essential

  • BSc/MSc physiotherapy
  • Professional Registration

    Essential

  • HCPC registered
  • Knowledge, Training & Experience

    Essential

  • Significant post registration experience including experience of working in all core clinical areas
  • Evidence of wide clinical theoretical knowledge base
  • Well-developed communication skills
  • Ability to work as part of a team
  • Experience of working as part of an integrated MDT
  • Evidence of supervising other staff and students
  • Flexibility
  • Good organisational and time management skills
  • Evidence of CPD
  • Understanding of professional standards and code of conduct
  • Basic IT skills
  • To demonstrate an awareness and understanding of supporting equality and valuing diversity within their role
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs
