    What is the Positive Futures Team?

    The Positive Futures Team is a small team of 3 members of staff (1 Social Worker and 1.5 Social Care Assessors) based within the Transition Adult Social Care Team (formally known as the Preparing for Adulthood Team).

    The TASC team is a social care team set within Integrated Adult Social Care, with strong links with Children's Services and Education. We work with young people from the age of 14 up until 25 across the Devon County Council footprint.

    The team comprises of two Team Managers, Social Workers, Social Care Assessors, Occupational Therapists and Business/Information Support Officers.

    The Positive Futures Team is a parallel team to TASC and primarily focuses on young people from 17 – 25 years of age who are leaving independent specialist education colleges and who require an assessment and planning for when their education ends.

    The team works to the Care Act principles to engage with young people with a disability ensuring that independent living skills are promoted and maximised to facilitate young people's ability to engage with their community, gain employment, and live and enjoy their adult life as independently as possible.

    This is an exciting time for the Positive Futures Team as we are approaching the second year of our TASC project. This project's overall aim is to improve our joint services in supporting young people in Devon as they plan and make decisions about what their adult life will look like.

    Over the first year of the project we have shown that effective collaboration with Children's Services, Education and voluntary agencies is improving the outcomes for young people, and those that care for them, as they transition into adulthood.

    We are delighted to announce that we are looking to recruit to a 2 Fixed Term 18 month contracts for Social Care Assessor positions within the Positive Futures Team.

    It really is a very crucial time in our project development and the successful applicants for these posts will play a significant part as both the Positive Futures Team and the TASC team continue to evolve. Being part of this project will require our team to demonstrate flexibility, adaptability, and a commitment to continuous improvement in service delivery.

    About You

    Are you a someone who has experience of:

    • working in a health, social care or educational setting?
    • working with individuals with disabilities?
    • working with children or young people?
    • Working in partnership with other agencies and teams to support positive outcomes for individuals?
    • Experience of undertaking assessments

    Such experience would be a great advantage for potential applicants for these Social Care Assessor positions in our team.

    We are ideally looking for one full-time SCA (37hours) and one part-time SCA (18.5hours) for these roles. However, if you can offer different hours, please can you advise of this in your application form and it can be discussed further at interview.

However, we pride ourselves on being a supportive and friendly team who communicates and supports each other on a daily basis. Additionally, we provide 1-1 supervision, monthly practice-focussed peer discussions and monthly team meetings. These are a mix of face to face and remote MS Teams meetings.
