Rehabilitation Case Manager
Full-time, Permanent (Part time would be considered)
00 - £45,000 per year plus pension and bonus opportunity

They are a well respected and long-established small case management business supporting adults and children who have sustained catastrophic injuries.

They work together as a team, centred on promoting the rehabilitation of their clients.

As a Case Manager you will be required to assess and understand a person's specific needs and use your clinical and professional skills to ensure maximum recovery.

Full training and support will be provided.
Undertake assessments and provide client centred reports including recommendations for rehabilitation.

• Work alongside private and statutory sectors to co-ordinate robust rehabilitation plans.
• Oversee the recruitment of support workers, providing training and ongoing supervision.
• Complete care plans and undertake regular risk assessments, ensuring that mitigating actions are in place.
• Travel within the Midlands area

You will require access to a vehicle and hold a valid UK driving licence.

• Flexibility to work from home or office.

* 28 days annual leave per year, plus bank holidays.
