**Permanent contract | £27,344 to £29,439 per annum (Scale G) | 23 hours per week | County Hall, Norwich

An exciting opportunity has arisen to work for Adult Social Services to support patients being discharged from the Julian Hospital.

We are looking to recruit an enthusiastic Assistant Practitioner to join our discharge team at the Julian Hospital.

You will manage a case load working closely with colleagues from Norfolk and Suffolk Foundation Trust (NSFT) and other partners during the assessment process to achieve timely discharges and reviews in line with the Care Act 2014.

You will work closely with Social Workers in the team. You will be working in partnership with a wide range of health professionals in a fast-paced environment. You will be passionate about making a difference, and ready to embrace change.

A team player, you will need to demonstrate an ability to work under pressure, be well organised and able to effectively prioritise workload.

Ideally you will have experience of working with Older People with complex needs and have either experience/interest in working within the mental health field of Social Care.

The ability to travel is essential.

You will have regular supervision and the support of a highly motivated team along with the relevant training required for the post and to develop your role.

Working remotely will be a feature of this role, when offices re-open your office base will be The Julian Hospital, Norwich.
