Vacancies have arisen within the Continuing Care Service. The service offers home care/community support to children with complex medical needs.

We are looking to recruit enthusiastic team leaders to join the Continuing Care Team.

The Continuing Care Team provides specialist support for children/young people with complex medical needs, in the community.

As a Team Leader you will be involved in the clinical supervision and ongoing support of our team of Senior Health Care Assistants.

The role will involve cascading training to other team members and therefore the successful applicant must be prepared to undertake further clinical training themselves.

Responsibilities also include planning, implementation and evaluating care in conjunction with parents, carers and other staff members.

You will be working with children and young people aged 0 -19 years with various levels of dependency. All the children/young people you will be working with have complex health issues/severe disabilities.

The service provides bespoke care for a wide range of conditions which range from ventilated dependent young people, terminally ill children and others with severe disabilities/additional complex health problems.
