    Job summary

    Trinity HealthGroup PCN is looking for Nurse Associate to support our Outreach Team. Thepostholder will work with our existing LTC Outreach Nurse forming an innovativeteam developing new ways of working. The postholder will focus on completingLTC reviews for housebound patients plus being a key member of the teamdelivering on our Health Inequalities Projects.

    Main duties of the job

    Trinity Health Group PCN has employed an Outreach Team which currentlyconsists of a Practice Nurse, Care Coordinator and support from a Health &Wellbeing Coach. The Outreach Team work closely with all member practices todeliver LTC reviews to patients that are housebound and in Care Homes.Additionally, we deliver targeted support to reduce Health Inequalities withinthe PCN. This includes setting up pop up clinics in areas of socialdeprivation, traveller sites and working with community/religious groups makingour services more accessible for patients.

    Qualifications

    Essential

  • Registered Nursing Associate on NMC Register
  • Nursing Associate Foundation Degree qualification level 5
  • Knowledge

    Essential

  • Basic understanding of General Practice environment
  • Experience

    Essential

  • Experience of working within a nursing or healthcare environment
  • Portfolio of learning in relation to knowledge and experience acquired whilst completing Nursing Associate course
  • Detailed knowledge and understanding of appropriate procedures and treatment plans
  • Ability to participate in reflective practice and clinical supervision activities
  • Ability to recognise own limitations in the context of the nursing associate role
  • Understanding the scope of the role of the Nursing Associate in the context of the team and the organisation, and how the role contributes to Quality Improvement
  • Desirable

  • Prior experience of working as a Nursing Associate within a primary care setting
  • Practical experience of using SystmOne
