Job summary


This is a new and exciting role where the post holder can use their communication and digital skills to help support the needs of the practices and patients that we represent by liaising directly with patients and link in with GP surgeries and a range of community health and social care services.

Our 2 Primary Care Networks provide new and additional health and wellbeing services to our patients.

Working in partnership with our 8 GP Practices, we deliver extra services such as wound care, physiotherapy, mental health support, wellbeing clinics, respiratory reviews, BP monitoring, frailty home visits, medication reviews and other innovations.


Main duties of the job


The post holder will provide telephone, administration, and facilitation support for multi-disciplinary team (MDT) meetings and to encourage greater uptake of PCN practice and community appointments.


The post holder will demonstrate excellent IT, organisational and communication skills, be flexible in their approach and able to exercise initiative and demonstrate consistently high standards of professionalism.

The post will support the PCN and its practices to deliver ongoing improvement to services.

The role will require experience in quality and service improvement with experience or a sound interest in delivering transformational change and have a good understanding of general practice systems and processes, including familiarity with general practice digital systems and how they can be used to deliver service improvements and better health outcomes.

MAIN DUTIES AND RESPONSIBILITIES


The post holder supports the PCN by attending meetings where necessary such as multi-disciplinary team (MDT) meetings, linking in with GP surgeries and a range of community health and social care services, Executive Meetings, ad Hoc PCN team meetings.


The post holder will demonstrate excellent IT, organisational and communication skills, be flexible in their approach, able to exercise initiative and demonstrate consistently high standards of professionalism.

The post holder must always be aware of the need for confidentiality and integrity.


The post holder will help set up clinics and maintain the PCN patient services booking platforms (SystmOne) and other PCN wide IT systems as and when required.

They will work closely with the PCN Leadership Team to manage the patient flow and digital inclusion. They will be proactive in rearranging clinics / appointments as appropriate to manage unplanned absences whilst updating relevant parties.

The postholder will seek to improve patient access to information, clinics, systems and services deemed appropriate to everyone.

The postholder will assist their PCN and practice colleagues with equipment and information when needed.


Workforce Responsibility

  • The post holder must remain up to date with mandatory training as required.

Environmental Factors

  • The post holder will be required to drive.
  • The post holder may be required to undertake duties at any location in the community to meets service needs.
  • Concentration required for data analysis, tracking patients and meetings, frequent interruptions requiring attention and reprioritisation of work.
  • Input data for a significant period

Hours:

You will work a 37.5-hour week, Monday to Friday.

Your hours need to suit the workload, mostly during normal office hours, however, occasionally early mornings or the odd evening and weekend work may be required for activities such as facilitating or attending meetings, PCN clinical events or carrying out operational on-call activity if the operations manager and/or business manager are unavailable.

Driving:
You will be required to use your own car for work, and to be insured accordingly. A mileage allowance will be paid for essential travel in connection with work from your main base.


Person Specification

Qualifications

Essential

  • Educated to GCSE or equivalent (GCSE Maths & English C or above)
  • Demonstrable commitment to professional and personal development

Desirable

  • NVQ 3 or equivalent and/or relevant basic/first level professional qualification, or working towards this

Experience

Essential

  • Customer service skills (F2F and telephone)
  • Good keyboard and computer skills
  • Understanding of, and commitment to, equality, diversity and inclusion
  • An appreciation of the nature of general practice
  • Ability to build and maintain relationships whilst maintaining appropriate professional boundaries
  • Excellent verbal communication skills with the ability to communic
