    Job summary

    Derbyshire Dales PCN(DDPCN) is a forward-thinking primary care network keen to embrace theopportunity to work with both established and Early Years Clinical Pharmacists todevelop a new pharmacy service by working in partnership with the 8 GPPractices that we support as part of the holistic health care services offeredto the 50,000 patients registered across the Central and North DerbyshireDales.

    We are looking toemploy 4 PCN Clinical Pharmacist to join our growing multi-disciplinary team.

    Main duties of the job

    To work as part ofa multi-disciplinary team in a patient-facing role taking responsibility forareas of chronic disease management within the practices.

    To undertakestructured medication reviews to proactively manage patients with complex polypharmacy,especially for older people and those with multiple co-morbidities.

    To provide supportto general practice staff with regards to prescription and medication queries.

    To help support therepeat prescriptions system and deal with acute prescription requests.

    To undertakemedicines reconciliation on transfer of care and contribute to the developmentof systems for safer prescribing, providing expertise in clinical medicinesadvice while addressing both public and social care needs of patient in the GPpractice(s).

    To proactively workalongside the wider PCN, MSK and Mental Health multidisciplinary teams, onprojects such as tackling health inequalities, low prevalence for CVD and highlevels of opiates.

    The role is pivotalto improving the quality of care and operational efficiencies so requires motivationand passion to deliver excellent service within general practice .

    About us

    DDPCN is based in Tansley, Matlock and support the 8 GP Practices in Matlock, Darley Dale, Ashover, Bakewell , Tideswell , Baslow , Eyam and Hope plus the many picturesque villages served by the practices .

    The office is in the heart of the Derbyshire Dales, but you will be working remotely most days within the practices.

    There is an excellent road infrastructure with access to the M1 Motorway, Sheffield, Derby, towards Manchester and Staffordshire are easily commutable.

    You will be joining a friendly, forward thinking PCN who with the 8 practices support approximately 50,000 patients, delivering high quality patient focused care.

    Job responsibilities

    To work as part ofa multi-disciplinary team in a patient-facing role taking responsibility forareas of chronic disease management within the practice.

    To provide supportto general practice staff with regards to prescription and medication queries.

    Clinicalpharmacists will work as part of a multi-disciplinary team in a patient facingrole to clinically assess and treat patients using their expert knowledge ofmedicines

    They will beprescribers, or will be completing training to become prescribers, and willwork with and alongside the general practice team.

    Clinical pharmacistswill undertake medication reviews to proactively manage people with LTC, peoplein care homes, frailty, and people with learning disabilities or autism

    ClinicalPharmacists will provide specialist expertise in the use of medicines whilsthelping to address both the public health and social care needs of patients atthe PCNs practices and to help in tackling inequalities.

    Clinicalpharmacists will provide leadership on person centred medicines optimisation(including ensuring prescribers in the practice conserve antibiotics in linewith local antimicrobial stewardship guidance) and quality improvement, whilstcontributing to the QOF and both practice /PCN enhanced services.

    Through structuredmedication reviews, clinical pharmacists will support patients to take theirmedications to get the best from them, reduce waste and promote self-care.

    Clinicalpharmacists will have a leadership role in supporting further integration ofgeneral practice with the wider healthcare teams (including community andhospital pharmacy) to help improve patient outcomes, ensure better access tohealthcare and help manage general practice workload.

    The role has thepotential to significantly improve quality of care and safety for patients.

    The ClinicalPharmacist will develop relationships and work closely with other pharmacyprofessionals across DDPCN and the wider health care systems.

    Clinicalpharmacists will take a central role in the clinical aspects of shared careprotocols, clinical research with medicines, liaison with specialistpharmacists (including mental health and reduction of inappropriateantipsychotic use in people with learning difficulties), liaison with communitypharmacists and anticoagulation.

    Take part inClinical Audit and quality improvement to ensure prescribing is in line withlocal and national guidance, CQC requirements and up to date MHRA /CAS alerts.

    Clinicalpharmacists as appropriate will support the Lead Clinical Pharmacist inmentoring providing training /support for the Pharmacy Technicians and PharmacyAdministrators

    Support the widerclinical team with education and induction programmes for new clinical staff

    Support the LeadClinical Pharmacist in ensuring appropriate representation at local ICBMedicine Management Team meetings and other PCN/practice meetings

    Person Specification

    Qualifications

    Essential

  • Master's degree in pharmacy
  • Audit and Quality Improvement
  • Proficient in use of Microsoft Office (including Excel, Word, PowerPoint, Teams and Outlook
  • Community or primary care / NHS experience
  • Desirable

  • Independent Prescriber (IP) or working towards qualification.
  • Be willing to undertake IP course if required as part of role requirement .
  • MDT Working
  • Knowledge and experience of using SystmOne and EMIS
  • Developing pathways or processes
  • Understanding of QOF, IIF and PCN DES
  • MHRA and CAS alerts
