    Job summary

    Are you an experienced Children's nurse looking for a new challenge or a nurse looking to further develop their skills within acute paediatrics? We are looking to recruit a team of band 6 nurses to pilot our Paediatric Virtual Ward.

    The Paediatric Virtual Ward will allow patients to get hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most and reducing the burden of travel for families.

    As part of this pilot you will be involved in developing the service to ensure that it meets the needs of our local population. From the beginning, your input will be pivotal in developing clinical pathways and operational processes.

    STAFF CAR PARKING

    Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 - 18:30.

    For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.

    This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete "on call" duties within their role.

    Main duties of the job

  • To be responsible for the triage of referrals, assessment of care needs and the development, and evaluation of programmes of care in the community, within Scope of Professional Practice.
  • To evaluate treatment interventions for the caseload and modify healthcare plans. To support CYP and their family's to be adequately supported in the home environment and to provide training, education and advice.
  • To work toward preventing inappropriate admission to acute hospital settings and to support timely discharge into the community.
  • To be responsible for the coordination of the day-to-day planning and delivery of Nursing services within the team, prioritising clinical needs and delegating where needed to provide an effective service.
  • Demonstrate high level of judgemental/analytical skills when interpreting and performing patient assessments and care planning via the telephone and face to face.
  • Maintain accurate and up to date patient care records, maintaining patient confidentiality, and statistical records as required by the service.
  • Work with other professionals to promote multidisciplinary / multiagency working. Being responsible with other senior colleagues to provide the total needs of the patient/client group, planning and delivering services.
  • To attend and contribute to MDTs
  • Communicate information to CYP and family's/carers with empathy and reassurance.
  • To maintain a patient led focus in all aspects of care and co-ordination.

    Education & Qualification

    Essential

  • RN Child (Part 1 of register) or equivalent
  • Current NMC registration
  • Education to diploma level or able to demonstrate evidence of post registration experience at Band 5 within an acute paediatric setting
  • Evidence of personal and professional development
  • Desirable

  • Independent/supplementary prescribing qualification
  • Consultation skills
  • Recognised HDU or Advanced Assessment skills Qualification
  • Experience & Knowledge

  • Multi-disciplinary working/Multi-agency working
  • Post registration practice within a secondary, community or primary care setting
  • Working knowledge of legislation related to Paediatric services
  • Basic working or in depth knowledge of NHS polices affecting hospital avoidance

  • Experience of working at Band 6 level

  • Be able to evidence skills and knowledge of clinically assessing and monitoring children and young people with acute illness.
