We are seeking a dynamic and highly experienced specialist Occupational Therapist or Physiotherapist with a track record of leading and managing a clinical team in a community setting within the Community Integrated Response Team (Home First, UCR 2hr Response and Virtual frailty Ward)


You will be providing line management and clinical/ professional supervision to a defined group of staff, including appraisal and monitoring of clinical practice standards and be responsible for the allocation of work to clinicians and working collaboratively with the other team leaders to ensure equity of workload across teams.


You will act as positive role model in coaching staff members and providing clear direction and support, as well as supporting the service manager with service re-design and take the lead on identified projects.

You will be in charge of managing a clinical caseload as capacity allows.


We are looking for someone who has exceptional communication, interpersonal and leadership skills, with the ability to take charge when needed and an encouraging and inclusive approach.

You must be flexible and adaptable with the ability to manage competing demands, priorities and deadlines of the role.

The team leaders will be required to drive the transformation of community therapy, so you need to be forward thinking and have a passion for community rehabilitation and quality improvement.

To lead and co-ordinate a multi-disciplinary team of therapists, assistants, health care workers and administrative staff to provide a
comprehensive, efficient and effective community-based therapy service.
To provide a high standard community therapy service to patients in their own living environment with the aim of
preventing hospital admission or supporting hospital discharge. To perform highly specialised therapy assessment of
patients with diverse and complex conditions to provide a diagnosis and develop and deliver an individualised treatment
programme.
To assess, develop and implement a Highly Specialist Occupational Therapy service for a highly complex client group
with a wide variety of clinical needs.
Lead in the development of the Community Integrated response Team in specialist area of Occupational Therapy and Physiotherapy.
To support the Clinical Lead in matters relating to planning and development of the service
The post holder will be expected to supervise Occupational therapists, Physiotherapists and Rehabilitation Assistants within the team and
have a key role in the development of integrated working for all AHPs within Community Integrated Response Team
To support the Service Lead in the delivery and development of a high performing service/ skilled and productive
workforce that is patient focussed.

The Community Integrated Response Team enables collaborative working of staff across Wirral Community NHS Foundation Trust.

It ensures that patients in crisis and at imminent risk of hospital admission are provided with a thorough MDT
assessment at the earliest opportunity, focusing on promoting independence, ensuring people are able to remain in their
own homes for as long as possible, without the need for hospital admission or long term care placements.

The team consists of Physiotherapists, Occupational Therapists, Technical Instructors, Nurses, Health Care Assistants
and Social Care Staff who work together to provide short-term support and rehabilitation in the patient's home. They have
access to specialist services and equipment, which may help keep the patient safe at home.

To lead a team of multi-disciplinary therapists including clinicians, assistants and admin staff to provide a high quality,
effective and productive Rapid Community Response Team.
To be competent with Information technology and to lead and monitor the team in the use of electronic patient
information services.
To monitor and provide reports on team performance and activity as required and support Trust initiatives to develop
and improve service efficiencies.
Ensure team compliance with information governance policy.
To regularly participate in working parties/groups as required developing policy and protocol changes across the Trust
that will impact on all service users and to implement those policies and protocols within the service.

Support the Service Lead in completion and analysis of accurate records of activity for statistical purposes and implement changes within the team as a result of audit.
