    Areyou passionate about supporting people with Learning Disabilities and committedto improving their quality of life? An exciting opportunity has arisen for an OccupationalTherapist to join the Wakefield Intensive Support Team (IST). This isa permanent part time role on at 30hrs per week.

    The IST is a small multidisciplinaryteam with a Psychologist, Nurses, SLT and Support Workers working proactivelyand intensively with individuals with behaviours that challenge. We providespecialist assessments and interventions to support service users to remain intheir home, and our work is informed by the principles of Positive Behaviour Support.We work closely with carers, support networks and wider agencies to developtheir understanding of service users needs.

    The IST is part of the CommunityLearning Disability Health Team, a friendly and welcoming MDT. There is an emphasis on multidisciplinary working withpositive relationships between professional groups. We also have strong andeffective links with our partner agencies including the Local Authority and ICBas well as local care providers. The team are committed to delivering theTransforming Care agenda and your input will be highly valued.     Main duties of the job

    You will bea highly motivated and conscientious individual who has the desire to workcreatively with people with learning disabilities within the community, providing the higheststandards of safe and compassionate care to clients to maximise theirpotential.

    Youwill be required to use your specialist OT skills to support the assessment anddelivery of high-quality therapeutic interventions to address the occupationalneeds of service users. This will require effective MDT working skills, closeliaison with other agencies and use of excellent communication skills.

    The successful applicant will manage acommunity caseload of service users who have a variety of presentations,enabling activity participation and independence in a community setting,including training of family carers and paid carers in different settings. You would be required to evaluatetreatment outcomes and contribute to service development.

    Further post qualification is not essential, as there will be opportunity to undertake an accredited qualification in PBS. Professional development is actively encouraged and supported.

    Job responsibilities

    JOB SUMMARY

    The post holder will be part of acohesive and enthusiastic multi-disciplinary team and work closely with theother parts of the locality health team and In-Patient team where required. You will be involved in providing specialist assessment,support and treatment for individuals receiving intensive support and care,enabling service users to engage in a wide range of individual and group based treatmentactivities.

    Thepost holder witll promote a positive behavioural approach which will beunderpinned by professional skills and additional training as required.

    Thepost holder will be part of a well recognised service that places emphasis ofstaff support and professional development and you will be expected toparticipate in staff training and supervision.

    KEY RESULT AREAS:

    Clinical

    1.To manage an individual caseload, providingassessments for service users receivingin-patient care and withinthecommunity, prioritising clinicalneeds and providing an effective service.

    2.To work with service users to identify goals aspart of the overall care plan, using appropriate assessment tools based on currentevidence based best practice.

    3.To plan and implement a client centredtreatment programme on an individual and/or group basis, to achieve therapeuticgoals.

    4.To monitor, evaluate and modify treatment inorder to measure progress and ensure effectiveness of intervention.

    5.To demonstrate a broad level of understandingof the effect of disability and provide training and advice on lifestyleschanges and adaptations to the clients social and physical environment.

    6.To be responsible for providing advice in themultidisciplinary team assessment of service users and carers to ensure that anintegrated approach to assessment and treatment is achieved and maintained.

    7.To work proactively alongside ward staff, communityagencies and voluntary agencies to provide a seamless service for service users.

    8.To maintain an effective communication networkwithin the team, and to liaise with other agencies and disciplines from thevoluntary and statutory sectors to ensure that service user needs are met.

    9.To maintain up to date service user records andactivity data in accordance with professional and Trust standards.

    10.To provide and contribute to multi-disciplinary team meetings, wardrounds, clinics and case conferences as required.

    11.To provide reports (as prequired) on assessmentand treatment by written and verbal reports to Consultants, GP's and otheragencies involved, and to ensure that all correspondence relating to caseloadis dealt with promptly.

    12.To empower families and carers to supportindividuals with learning disabilities.

    13.To involve clients in all aspects ofassessment, treatment and evaluation of treatment programmes.

    14.To promote the positive health and well beingof the individual.

    Managerial

    1.To manage own caseload and day-to-day activities,managing and responding to patient crisis in a timely manner and as perprotocols of the service.

    2.To co-ordinate the day-to-day activities of juniorstaff in the service area.

    3.To provide effective guidance and supervisionfor junior staff.

    4.To take responsibility for risk managementwithin the service area.

    5.To co-ordinate statistical collections in line withTrust policies.

    6.To ensure responsible management of Trust equipmentand supplies.

    7.To participate in the recruitment, appointmentand induction of staff, ensuring that practice in these areas areanti-discriminatory.

    Leadership

    1.To promote the benefits of the service to thepeople and organisations affected by it.

    2.To work effectively as a team member, sharingskills and knowledge and collaborating with other team members.

    3.To act as a role model to all staff,demonstrating and encouraging healthy working relationships.

    5.To promote awareness of the role of theIntensive Support service within the team, negotiating priorities whereappropriate.

    6.To participate in development and improvementof the Learning Disability service and develop evidence-based practice in theservice.

    7.Ensure the Operational Manager/IntensiveSupport Lead is kept informed of all aspects of the work, especially thedevelopment needs of the service.

    Policy and Service Development

    1.To contribute to the appraisal of LearningDisability services and to identify appropriate developments in the servicesprovided making recommendations to the Intensive Support team Lead/OperationalManager.

    2.To contribute to agreed developments in Learningdisability service provision.

    3.To be responsible for ensuring that thepolicies and procedures of the relevant Codeof Professional Ethics and Professional Conduct and local and nationalstandards of practice are properly implemented.

    4.To take a role, in collaboration with othersenior staff, in the development of services, including operational planning,and implementation of policy and service development.

    5.To be aware of national guidelines andlegislation relating to health and social care in your specialist service area.

    Qualifications

    Essential

  • RNLD or BSc/BA, MA Occupational Therapy
  • Additional post grad qualification in positive behaviour support at Diploma/Degree level
  • HCPC State Registration/NMC registration
  • Desirable

  • Evidence of additional academic study at Diploma/Degree level

  • Experience

  • Substantial post registration experience working in /learning disabilities
