    Post: Band 6 Physiotherapist - Specialist Palliative Care

    Hours: Full time, core hours Monday - Friday

    Salary: Agenda for Change, £35,392 - £42,618

    We are seeking to recruit an experienced, competent and motivated physiotherapist to join our new specialist palliative care therapy team based at St John's Hospice in Doncaster.

    This role will cover specialist physiotherapy input for patients within the St John's Hospice Inpatient Unit, the St John's Hospice Day Therapy Unit and outreach within patient homes in the community.

    This role will focus around embedding a rehabilitative palliative care model using a person-centred approach. Key elements of the role will include mobility assessments, falls assessments, activity/exercise advice, symptom self-management advice, holistic assessments and signposting for patients with a life-limiting, palliative condition. This may be completed on a 1:1 basis or within group sessions in the Day Therapy Unit. Experience and a keen interest in the palliative care speciality is essential for the successful candidate.

    Main duties of the job

    To provide specialist assessment and treatment of patients with complex therapy needs as an autonomous practitioner within the specialist palliative care team.

    To manage own caseload using evidence based / patient centred principles to assess, plan, implement, evaluate, and record all interventions.

    To provide ward based physiotherapy within the inpatient unit, including mobility assessments, falls assessments, provision of mobility aids, exercise programmes, discharge planning.

    To provide physiotherapy input within the Day Therapy Unit, delivering a range of patient group sessions. This includes exercise, education, social, guided activities.

    To provide education and promotion of self-management of symptoms within palliative conditions.

    To integrate a rehabilitative palliative care model into practice.

    To outreach to community patients in their own homes to provide specialist palliative care input as required.

    To provide one to one clinic sessions where patients require a more intensive approach to care.

    To complete initial assessments in clinics, ensuring a holistic approach and appropriate planning of care.
