    Description

    The Mental Health Triage and Response team is a dynamic new service which incorporates the Parliamentary Award winning Street Triage Service and the Places of Safety which fall within the Merseycare Footprint.


    It will play an important role in Merseycare's response to the national roll out of Right Care Right Person across the region.

    Part of this development is an expansion of the current provision alongside Northwest Ambulance Service.


    We are looking for experienced practitioners and this role is not suitable for individuals who are newly qualified or are yet to qualify.


    The role involves working in collaboration with Police and Ambulance colleagues at a variety of bases and in the community.

    The service works towards the vulnerability model supporting people with mental health issues, substance misuse and safe guarding needs. The post holder(s) needs to demonstrate the ability to effectively recognize, assess and manage risk in an autonomous role.


    Another aspect of this position is supporting Emergency Services and service users at the point of crisis via a dedicated telephone service.


    The core role includes face to face assessment, specialist mental health assessments, supporting and sharing information with the wider emergency services community and acting in an advisory capacity in conjunction regards to mental health and capacity law.

    Flexible working requests will be considered for all roles.
    Participate in regular clinical and managerial supervision as per Trust policy.

    Participate in supervision to monitor standards of service user care and documentation.

    Assist with screening and telephone triage of referrals, prioritizing as appropriate. Formulate, contribute and participate in the development and implementation of specialist assessments, including risk, with patients requiring crisis care. Formulate and deliver plans of care as appropriate in collaboration with the multi- disciplinary team

    Carry out generic and specialist assessments including risk with patients requiring crisis care.

    Formulate and deliver plans of care as appropriate.

    Provide short-term evidence based practices and fixed term interventions

    Support the delivery of education and self-help materials within a variety of locations.

    Assist in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.


    Ensure the clinical environment is conductive to promote service users health, safety and well-being, thereby preserving and respecting each services users personal dignity, privacy, religious and cultural beliefs.


    Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC Code of Professional Conduct and Trust Medicines Code.

    Participate and promote positive intra-agency working.

    Participate and assist in setting standards of service user care based on sound research findings and evidenced based practice.
    Evidence of post registration study / continuing professional development e.g. Significant demonstrable post registration experience in mental health
    Experience in change management and policy formulation.

    Evidence of experience of working with a range of people with mental health problems in a variety of health care settings.

    Being responsive and flexible, seeing change as an opportunity to improve services
    Proven knowledge of relevant mental health legislation.
    Demonstrate knowledge in the concepts of mentorship, preceptorship and supervision and ability to provide education and supervision to junior staff
    Ability to contribute to developing care excellence within the ward / team area
