This is an exciting opportunity for a star workers within the Early Intervention for Psychosis (EIP) teams in the Hammersmith and Fulham.

We are looking for a highly motivated individual with a proven experience of working in a community based setting.

You will be responsible for the delivery of care to an allocated caseload of Service Users, living in the Community, experiencing, or recovering from, mental health difficulties, specifically focussing on Early intervention principles.


You will provide direct support and advice to Service Users and Carers, within the Early intervention framework, embracing the individual right to choice and control and in accordance with Clients' Care Plans.


You will work as part of a multi-disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust.

The post holder will be required:
To work within the multi-disciplinary team structure of the Adult Early Intervention in Psychosis Team (EIS).

To support and work alongside the Care Co-ordinators and to assess, plan, implement and evaluate individual programmes of care.


To provide S upport, give T ime to an allocated group of service users, promoting R ecovery and maintaining them in the community environment.

To make a valuable contribution to multi-disciplinary team assessments and the formulation and implementation of MDT care plans.


To maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
