Contract:
Permanent, 25 hours per week.

Salary:
£20, pro rata, £10.25 per hour


Based at:
Northgate House, Pembrokeshire.


We are currently recruiting for a compassionate and organised individual to provide high quality and effective housing related/floating support service for individuals experiencing substance use issues across the County of Pembrokeshire.

The support will focus on an individuals strengths enabling them to feel safe and secure with the long-term aim of them living substance free and independently in their own homes.

The post holder will work with service users in areas such as employment, training and education, debt management and daily living skills to promote wellbeing and a healthy lifestyle.

You will be expected to be flexible and work across the hours and days of service delivery and will be part of a rota of on-call service for out of hours provision where necessary.


Main duties/responsibilities:


  • Provide a variety of person centred, tailored interventions that support individuals with complex needs to find sustainable accommodation and develop their independent living skills.
  • Working with service users to impact positively on issues relating to housing, employment, training and education, offending behaviour and social isolation.
  • Manage a client caseload, and complete individual needs assessments, risk assessments, support plans and offer practical advice and advocacy to service users.
  • Support service users to exercise informed choices about their lives where possible and to enhance their social skills and participate in their community.
  • Support service users to identify the areas of support needed and identify the best option of support based on ongoing needs as outlined in their individual Support Plans.
  • Liaise with relevant services, organisations and agencies with regards to accessing additional support for service users.
  • To maintain up to date case management and review records as required.
  • Communicate clearly, effectively and professionally with service users receiving support, Support Workers, Carers, family members, referrers and other Professionals via multiple formats (i.e., facetoface, telephone, in writing etc.)
  • Signposting/referring on to other specialist/appropriate services.

PERSON SPECIFICATION:

Must have a full and valid UK driver's licence.


Essential qualifications:
There must be evidence of commitment to personal and career development relevant to the post.
Qualified or willing to work towards NVQ 3 in Health and social care or hold equivalent qualifications.


Essential Experience:
Experience of working with vulnerable client groups.
Experience of working in community-based services.
Experience of working with people with housing difficulties.
Experience of partnership working.
Experience of different treatment models to support the service user.
