An opportunity has arisen in the York Outreach Recovery Team for a Senior Mental Health Practitioner to work within the 'out of area' team which is devoted to the repatriation of service users from in patient rehab hospitals back into their local area in the community.

Applications are invited from those with a relevant nursing qualification or allied professional qualification, who has the ability to work independently and within a team.

The post holder will be responsible for supervised care co-ordination and co-working of identified clients in conjunction with private sector partners.

It is vital that the applicant has previous experience working mental health

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes.

If necessary, adjustments can be considered in accordance with the Equality Act 2010.
