We are currently looking for a Health and Social Care Officer to work within the front door to adult social care processing and triaging referrals.

The postholder will provide advice, triage and co-ordinate information to enable decision making and risk managementon behalf of citizens requesting / requiring access to social care and health services, and to navigate citizens through to the right services based on an assessment of their needs.

To assist in the delivery of an efficient and effective referral service for vulnerable adults sometimes with complex needs and/or difficult and unpredictable situations.

Having a good understanding of the variety of services that are available to meet theindividual's needs.

Principal duties and responsibilities

To coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families and carers and to manage these in an effective, timely and customer focused way.
To deliver an agreed level of customer service that reflects the vision, values, aims and objectives of Nottingham City Council and partners.
To manage specialist or escalated calls from Health and Social Care Support Co-ordinators, Health and Social Care Support Advisors or other colleagues.
To have an understanding and knowledge of the variety of relevant services and their referral criteria available within social care and health to enable appropriate advice and guidance is given and to ensure this information is kept up to date.
Liaising with and working with relevant clinicians, practitioners and health and social care provider services to coordinate appropriate care packages and ongoing care needs, this will include triaging and signposting citizens to other services, trackingand monitoring pick up and handover of clients to the crisis response team.
