Healthy Horley Primary Care Network (PCN) has an exciting opportunity to expand their Primary Care Workforce by employing a Primary Care Network Coordinator to support the Clinical Directors and PCN Development Manager in the development of the Primary Care Network.


Our PCN comprises of 3 practices, covering the Horley Locality and includes Birchwood Medical Practice, Smallfield Surgery and Wayside Surgery.

The Practices have a combined population of 31,000 patients.

Healthy Horley PCN work closely with Growing Health Together, Alliance for Better Care and the Horley Health Hub, and provide services such as an Anticipatory Care Hub, Extended Access, Bereavement Counselling (in partnership with St Catherines Hospice), Community Garden, Community Wellbeing groups, and many more.

This is a part time post of 20 hours per week.


Main duties of the job


This role will provide operational and administrative support to the PCN Practices to enable them to deliver the PCN Direct Enhanced Service Framework (DES), achieve to their best potential on the Impact and Investment Fund (IIF) indicators, progress through the Maturity Matrix and meet the strategic aims set out by the Practices.

You will work with Alliance for Better Cares PCN Development Manager and PCN team to ensure the PCN Additional Roles are well supported on a day-to-day basis and the Network is able to make full use of their potential.

The successful post holder will be supported by the wider ABC team to ensure the Primary Care Network is able to develop and thrive.


The post holder will work with the PCN Board and Network Practices to help develop and embed joint systems of working both between Practices and with other system providers (i.e., acute, community, mental health and third sector).

This role aims to provide on the ground presence with links to the full services that ABC offers to PCNs such as:

  • Recruitment and Employment
  • Project Management
  • Strategic Planning and Support
  • Leadership and Team Development


ABC will also support the postholder by sharing national guidance and best practice and linking them to the wider network of PCNs across East Surrey, West Sussex and Crawley.

Job responsibilities

Communication across the Network

  • Support effective communication between the Network Practices, and external organisations, and maintain regular contact with the PCN Practice Managers.
  • Share best practice and key documents with other Network Coordinators in Surrey and Sussex.

Production and delivery of the Network Business plan

  • Attend annual strategy sessions.
  • With support from the Alliance for Better Care team, produce and work towards an annual business plan linked to a longerterm strategic plan that includes both DES contract requirements and local projects. The postholder will also need to ensure the business plan is regularly updated and can be reviewed by the whole PCN.

Monitoring the performance against DES/IIF specifications

  • With support from the Alliance for Better Care team, work with the PCN board to ensure that Practices are ready to deliver the DES requirements as they come into effect.
  • Develop and use tools for gathering data and monitoring performance of the PCN.
  • Develop a good understanding of the DES requirements and the Impact and Investment Fund (IIF) indicators.
  • Monitor the PCNs progress of the IIF/DES and share updates with the PCN Board.
  • Ensure feedback loops in effect where IIF/DES performance is at lower than desired level to maximise achievement.

Project Managements of Identified Projects

Planning of projects:

  • Establish an initial project brief, asking key questions of those involved and doing background research.
  • Identify stakeholders and build effective working relationships with individuals and groups.
  • Present in a clear format that can be understood by all stakeholders to ensure buy in and engagement.
  • Work effectively with other team members to estimate time and resource required for delivery and how these fit in with other priorities.
  • Set measures against which the success of the project will be assessed.
