- Participatesin provider oncall rotation asrequired.
- Under thesupervision of the Executive Director the Medical Director providesgeneral medical direction and supervision of the medical aspects ofparticipant care.
- Responsible for the deliveryof participant care clinical outcomes and oversight of the QualityAssurance Performance Improvement (QAPI)program.
- Responsible for the development andmaintenance of medical policies and procedures/standards andprotocols.
- Provide direction and supervisionto related Primary Care Providers (PCPs) and Advanced PracticeProviders (APPs).
- Conduct history and physicalon new participants and eventbased and periodic reassessments.Develop medical treatment plan and provide medically necessaryservices in the range of locations in which participants mayreside.
- Interview and evaluate applicants toPACE Organization and their caregivers and participate in theIntake and Assessment decision.
- Discuss eachparticipant s medical conditions and prognosis with him/her andtheir caregiver to assist them in expressing their wishes andplanning their future care.
- Execute assignmentsin a culture that is shared and collaborative across all divisionswithin TH PACE.
- Collaborate in the nursingtreatment program of participants.
- Establishand/or maintain as a member of the leadership team a plan toachieve operating goals.
- Participate as anactive member of PACE Organization interdisciplinary team. Educatesand advises the team regarding the implications of diagnosescognitive and physical function and treatmentoptions.
- Participate in team and familyconferences and decisionmakingprocesses.
- Educate participants and familiesregarding health maintenance and their chronic medical conditionsin cooperation with nursing staff.
- Supervisethe health care and treatment program as developed by theinterdisciplinary team.
- Perform physicalexaminations order and evaluate appropriate laboratory anddiagnostic tests and record results in a systematicmanner.

Program ofAllInclusive Care for the Elderly (PACE) model is centered on thebelief that it is better for the wellbeing of seniors with chroniccare needs and their families to be served in the communitywhenever possible.
PACE serves individuals whoare age 55 or older certified by their state to need nursing homecare able to live safely in the community at the time of enrollmentand live in a PACE service area. While all PACE participants mustbe certified to need nursing home care to enroll in PACE only about7 percent of PACE participants nationally reside in a nursing home.If a PACE enrollee needs nursing home care the PACE program paysfor it and continues to coordinate the enrollees care. We encouragefellowship and independence while serving seniors and theirfamilies with dignity and respect. Delivering all needed medicaland supportive services PACE is able to provide the entirecontinuum of care and services to seniors with chronic needs whilemaintaining their independence in their home for as long aspossible.
