    Job summary

    An exciting opportunity has arisen for an enthusiastic and forward-thinking Band 6 Nurse to join the Transfer of Care Hub (Integrated Discharge Team).

    The successful candidate will promote the Discharge to Assess model by facilitating safe and timely discharges for patients from MDGH, supporting the acute medical ward and and as necessary for patients in community Pathway 2 beds transfer to pathway 3.

    The successful candidate will have a passion for putting the patient at the centre and have experience of working collaboratively with multi-professional partners.

    Hours per week: (part time would also be considered)

    Main duties of the job

    To coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital supporting proactive reduction in delayed transfer of care.

    To provide assessments for discharge pathways 1-3 and fast track discharges.

    Working proactively within an integrated health and social care team, to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.
