CLCH are recruiting Band 4 Nursing Associate roles on behalf of North Hammersmith & Fulham (H&F) Primary Care Network (PCN).

These roles give the opportunity to rotate across primary care and district nursing teams and gain a robust breadth of experience.

They are innovative opportunities which will give you experience of working closely alongside General Practice and District Nursing colleagues, developing your clinical skills and competencies and broadening your experience within a wide field of expertise.

They are also excellent career opportunities - we have a professional development pipeline from Nurse Associate to Registered Nurse and beyond.


Previous NHS or primary care experience is essential and applicants will need to demonstrate excellent communication and networking skills, self-motivation and a passion for holistic care that encompasses physical, emotional and social elements.


Working as a Nursing Associate you will need to be passionate and enthusiastic about patient care, putting patients at the heart of everything you do.


You will be working in multi-professional teams and will need to be able to think on the spot and be empathetic.


For these roles you will need to be able to work as part of a team and communicate effectively with other staff and patients.

You will be passionate about our business, creating a culture of continuous improvement, where patient welfare is at the heart of all you do.


It will be your responsibility to ensure that your behaviour and values reflect those of CLCH.The post holder will organise and prioritise their own day to day work tasks and activities, and delegate tasks to junior colleagues.


These posts are fixed term contracts until 31st March 2024 and there will be a requirement to work a weekend day and an early evening approximately once a month.

These roles offer full-time and part-time opportunities.


Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.


  • Assessment and planning of delegated care
  • Monitoring of longterm conditions and wellbeing of service users including escalation of interventions when necessary
  • Assist with the implementation and monitoring of clinical care and monitor outcomes
  • Administering of Medication in accordance with regulatory guidelines for Nursing Associates
  • Undertake onward referrals to appropriate services and community based care support
  • Assist in specific therapeutic relations to meet the physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers.
  • Enter data into clinical systems (patient records) as per Trust guidance and policies
