Our Private Patient Unit is growing and an exciting opportunity has arisen to join our friendly Private Patient Unit at Guy's and St Thomas' Hospital as part of the Patient Pathway Coordinator team.

Howard Ward

On Howard, we care for a variety of patients. Our main patient cohort is surgical and medical. The 9 bedded inpatient unit provides ensuite facilities. A medical officer is based on the ward and provides 24/7 support to consultants and the nursing team.

Our unit is based at St Thomas' Hospital in central London, one of the most prestigious and respected teaching hospitals in the UK.

We are expanding the service and it is an exciting time to join us. We are looking for a compassionate, enthusiastic and committed ward clerks, to join our new team.

Working hours (but not limited to): 4 days a week 08:00-18:00, including 2 weekends per month.

  • Job Summary
The Patient Pathway Coordinator will lead on the tracking of pathways of patients.

By working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met.


The post holder is responsible for providing a high standard of professional and patient focused service to a range of Trust-wide specialities and will support the function on a day-to day operational basis.


  • Key Relationships


The post holder will have regular contact with a wide range of clinical and managerial staff, administrative and clerical support staff at many levels and patients, relatives and external agencies.

Responsibilities:


The duties and responsibilities listed below are representative of the Patient Pathway Coordinator role and its purpose within the service; they are not exhaustive and the post holder may be required to undertake additional or alternative administrative and responsibilities commensurate with the level of the post, to support the smooth running of the department.

Operational

  • Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies (iPIM/Compucare).
  • Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review). Coordinating patient care e.g. liaising with theatre coordinator/ discharge lounge/ pharmacy. Assisting ward team with patient discharges. Liaising with multidisciplinary team to facilitate patient care pathway (e.g. confirmation of district nurse referrals, chasing social services/discharge team).
  • Receive patient referrals and administer in line with timings and requirements as set out in Trust and National Supporting relevant teams with the training and guidance provided.
