    Description

    We have an exciting opportunity for a Lipid Practitioner / Specialist Lipid Pharmacist to join the Diabetes and Endocrinology service.

    We are looking for someone on a full and part-time basis.

    Our Clinic based at Yeovil District Hospital provides support to patients with Diabetes, Endocrinology and weight management.


    Improve the care and outcomes for people with and at risk of cardiovascular disease (CVD) by supporting primary care towards detection and management of hyperlipidaemia (including Familial Hypercholesterolaemia - FH).

    Work in the community to identify and treat people at risk of and with pre-existing CVD.

    Search primary care databases, contacting individuals for intervention, counselling, treating and following up, referring onwards to the lipid clinic as appropriate.

    Identify people with possible Familial Hypercholesterolaemia and refer to the lipid clinic

    Work across divisional, organisational and professional boundaries, encouraging inter-disciplinary and inter-agency working for the benefit of improved patient outcomes.

    The precise details of service delivery will vary across differing practices and primary care networks (PCNs).

    Full current registration with the General Pharmaceutical Council (GPhC) or Nursing Midwifery Council (NMC) or Health and Care Professionals Council (HCPC)
    Demonstrable clinical pharmacy experience
    Community experience in pharmacy and or Nursing.
    Experience of patient-orientated medicines systems.
