    The Community Dietitian Team at Stockport NHS Foundation Trust have an exciting opportunity for an experienced Dietitian to provide maternity cover for an already established Frailty post providing dietetic support and service development for this vulnerable group of the population.


    We would welcome applicants with extensive Band 5 experience or Band 6 Dietitians who are looking to gain experience in project management and frailty.


    We are looking for an enthusiastic and motivated Dietitians to join our Integrated Nutrition and Dietetic service at Stockport NHS Foundation Trust.

    Work as an autonomous specialist dietitian to provide high quality service to agreed standards
    To provide advice on the use of oral nutritional supplements, enteral and parenteral nutrition as required
    Provision of expert advice on the management of patients referred to the nutrition team due to additional complex clinical and nutritional needs and liaise with Team Leader and Specialist Dietitians within the department to agree appropriate management
    To attend ward rounds, MDT meetings and best interest meetings as needed, promoting an understanding of the role of diet and nutrition in achieving the best outcome for each patient
    Provide training to healthcare professionals regarding nutritional care and interventions, including presentations

    Be responsible for planning, coordinating, delivering and evaluating clinical case work within the Frailty patient group

    Provide a key role in partnership working with patients, carers, the MDT, and wider statutory, private and voluntary sector services.

    To take responsibility for staff and student training and development.
    To provide evidence-based care, education and information to patients, carers and health care professionals. This will be in a range of inpatient, outpatient settings and community home visits.
    To take a lead with quality improvements in the Frailty Service

    oBSc in Human Nutrition and Dietetics (or equivalent qualification)
    oRegistered dietitian - Health Professions Council

    oValidated post-graduate training in nutrition support
    oSpecialist knowledge across a range of nutrition and dietetic procedures acquired through degree and postgraduate experience
    oKnowledge of NHS policies
    oAwareness of dietetic products and clinical reason(s) for use expert clinical practice in rehabilitation and frailty/malnutrition
    oAwareness of cultural diversity and equality issues

    oGood working knowledge of frailty and underlying health conditions

    oPost-graduate experience in nutrition and dietetic care of patients
    oExperience of training healthcare and other professionals

    oIT skills (nutritional analysis, word processing, email, inter/intranet searches.
