Role Overview
Oy client based in Belfast are seeking a compassionate and dedicated Care Coordinator to join their team.
As a Care Coordinator, you will be responsible for coordinating and managing the care of their clients, ensuring that their needs are met and that they receive the highest quality of care possible.
You will work closely with clients, their families, and a team of caregivers to develop care plans, manage schedules, and provide support to ensure our clients receive the care they need.
Responsibilities:
- Develop care plans and coordinate care services for our clients
- Communicate with clients, their families, and caregivers to ensure that care needs are met
- Manage caregiver schedules and ensure that all shifts are covered
- Provide support and guidance to caregivers to ensure that they are providing highquality care
- Conduct assessments of clients to determine their care needs and adjust care plans as necessary
- Monitor the health and wellbeing of clients and report any changes or concerns to the appropriate parties
- Maintain accurate records of client care and ensure that all documentation is up to date
- Ensure that all care services are provided in accordance with applicable laws, regulations, and industry standards
Requirements:
- Minimum of 1 years of experience in care coordination or a related role as a Senior Healthcare worker
- Strong communication, interpersonal, and organisational skills
- Knowledge of healthcare regulations and industry standards
- Ability to work independently and as part of a team
- Excellent problemsolving and critical thinking skills
- Proficient in Microsoft Office and other computer software programs

£19,500.00 per year
