    Job summary

    An exciting opportunity has arisen within Southern Health Foundation Trust to appoint an enthusiastic Registered Nursing Associate or Associate Practitioner (Band 4) to work within the multi-disciplinary Integrated Care Team in Chandlers Ford ICT.

    We are looking for individual who would like to be part of a dynamic and forward-thinking team... Do you enjoy the challenge of a wide variety of work? Could this be you?

    The role will include visiting patients at home to provide nursing interventions under the direction of registered nursing colleagues. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions.

    The post would suit someone interested in integrated working and wanting to achieve the best for their patients. They will work within a multi-professional team to provide individualised patient care.

    Main duties of the job

    We are looking for a motivated individual, who is dynamic and keen to work as part of a team. The successful candidate will need to have good communication skills and the ability to work well as a team member.

    In return Southern Health NHS Foundation Trust will offer training opportunities to support ongoing career development.

    Applicants must have access to a vehicle for work and possession of a full driving licence is essential.
