Description
Support Planner - IslingtonAre you a passionate, dedicated Support Planner looking for your next role? 4Recruitment Services are recruiting a Support Planner to join a Safeguarding and Family Support Team based in Islington.
70 per hour.Monday to Friday, 9am – 5pm, 35 hours per week.
Assist the Social Workers of the Disabled Children's Team in developing support plans for children and young individuals under their care within the Disabled Children's Team.
Create support strategies for children, youth, and their families in collaboration with both the family and professional networks.Occasionally, you may need to participate in and contribute to Team Around the Child meetings and Child in Need Reviews.
Ensure that objectives are SMART and tailored to the individual child/young person and their family.Ensure families comprehend Personal Budgets clearly, including their options for managing them, such as Direct Payments.
Arranging and determining the cost of support plans as needed.
Occasionally, meeting with families to assist them in establishing their support plan as necessary or suitable.
Calculate and organize payment arrangements for support plans.
Revise support plans in event of crisis or changes in family circumstances, including recalculating the costs associated with the support plan.
Experience in creating personal budgets and support plans for disabled children and their families.Experience in collaborating and communicating with disabled children and their families, along with an understanding of the challenges they encounter in a culturally diverse urban setting characterized by significant deprivation issues.
Capability to comprehend the requirements of parents and disabled children and facilitate other professionals in aiding these families through a motivational practice and relationship-centered approach.
