    The community is the place to be Rehabilitation in the community is at the forefront of the implementation of the Home First model.

    This exciting opportunity within intermediate care will provide you the opportunity develop skills and work as part of a dynamic multidisciplinary team with the opportunity to work with patients in different settings.


    This role is within the Intermediate Care Service - Inpatient rehabilitation (pathway two) where you will work within a multidisciplinary team to rehabilitate patients in an inpatient rehabilitation ward setting.

    You will have the opportunity to follow-up patients in their own homes following discharge and to support colleagues within the community based intermediate care teams (pathway one).


    The role is based at the Cambridge Rehabilitation Unit, Brookfield's Hospital and you will work closely with your colleagues in other localities and across the system that provide Intermediate Care for the population of Cambridgeshire and Peterborough.

    Intermediate Care strives to deliver outstanding patient-focused specialist care and therapy which collaboratively optimises a person's potential to achieve their individual goals.

    provide assessment and occupational therapy interventions to patients admitted for rehabilitation from acute hospitals and from the community.

    implement therapy plans as well as delegating to therapy assistants and integrated care workers.


    develop your skills in sub-acute rehabilitation and discharge planning working with ward based MDT and colleagues from other pathways, social care and the voluntary sector.

    work autonomously in a variety of settings to include travelling across the community completing access visits and seeing people in their own homes

    Degree/ equivalent qualification in Occupational Therapy
    Documented evidence of own continuing professional development

    Broad range of undergraduate experience in a variety of settings

    Clinical experience working with older people and long-term conditions

    Able to demonstrate commitment to providing high quality care
    Seeks and uses professional support appropriately.
