- Work closely with primary care network clinical directors and take direction from them as to the needs of the service.
- Support the PCN GP practices with care home workload and liaise with care home lead at practices.
- Work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating both drug and non-drug-based treatment methods into management plan.
- Refer care home patients appropriately in timely manner, such as acute admission, referral to community team or secondary care.
- Perform and interpret ECGs and perform investigatory procedures like phlebotomy service, swabs and doppler etc.
- Prescribe and review medication for therapeutic effectiveness in accordance with evidence-based practice, national and practice protocols.
- Proactively manage care home patients to increase the quality of care provided to this vulnerable group of patients.
- Accurate recording of data to agreed standards in patient records.
- Ensure high standard of clinical practice is maintained based on current best practices, embarking upon further training if required and continuing to update clinical knowledge
- Develop innovative concepts, models, methods, and practices which deliver new and improved primary care services to meet the changing needs of primary care for care home patients.
- Undertake audit and research when required.
- Promote healthy lifestyles, health promotion and disease prevention as applicable to care home patients. About the Candidate If you are an experienced Paramedic wanting a new challenge, we would love to hear from you. You will have the following qualifications, skills and experience:
- Degree in Paramedic Science or equivalent.
- Have completed or currently engaged on MSc level Advanced Clinical Practice education.
- Professional registration with HCPC and no current issues under investigation.
- Driving licence.
- Have completed Level 3 Safeguarding (Children).
- Knowledge of the implementation of Care Quality Commission standards.
- Can demonstrate a broad base of competencies and skills.
- Broad based experience of IT Systems including MS suite of applications.
- Knowledge of NICE guidelines.
- Understanding of legal and ethical issues/responsibilities relating to clinical practice.
- Knowledge of current prescribing issues.
- Knowledge of own professional accountability and autonomous practice.
- Understanding of infection control.
- Ability to evidence a sound understanding of the NHS principles and values.
- Possess substantial post-registration clinical experience.
- Have evidence of experience working in an Advanced Practice role.
- Able to demonstrate an advanced level of clinical practice within professional scope of practice.
- Evidence of working across organisational boundaries within health and social care.
- Experience of senior level decision making and delivery in a changing environment.
- Able to understand and analyse complex issues and balance competing priorities in order to make difficult decisions.
