We are currently recruiting for a Complex Care Manager for one of the UK's leading complex care at home providers, working partly remote with teams operating in the
Bolton and Prestonareas.

Joining this large community complex care provider, you will be responsible for coordinating a team of support workers in the delivery of complex care packages within the community.

Purpose of the role:

Coordinate the setup of a new client package, including undertaking the initial assessment meeting with the client to determine their needs. (Upon either on discharge from hospital or transfer from another provider).
Prepare monthly rotas in a timely manner for the care assistants and support workers, and ensure all clients receive service as designated in their care plan.
Build and maintain effective working relationships with key members across the business, including the OM, clinicians, other CCM's within the service and direct reports.
