Description

We require 2 experienced social workers to join our Discharge to Assess team. We are a small team made up of Social care, Occupational therapists, physiotherapists and a technical instructor. The team are based across 2 residential/ nursing home sites with 29 beds in total. The main aim to assess as required to get people back to their own homes if possible.

The job mainly entails working from a work base across both sites and working from home
