    Job summary

    An exciting opportunity has arisen to work within theDiana Childrens Community Service as a Band 7 CCT Clinical Team Leader.This team delivers support to C/YP with complex health careneeds across LLR. Care packages can include home based respite delivered bya team of competent Band 4 Specialist Practitioners, NA's and Nurses.

    We are looking to recruit an individual withexcellent partnership, networking and communication skills who has clinical and operational management experience, strong collective leadership skillsthat can motivate individuals and teams to build and sustain change.

    The candidate will demonstrate anunderstanding of working in the community, and the complexities of workingwithin a family as well as a multidisciplinary, multiagency environment; befamiliar with children's complex health needs and the importance of trainingand delegating clinical competencies. The successful candidate will need topossess the clinical competencies to lead the team ensuring safe care is alwaysdelivered. You will also be part of the Diana on-call team providing out ofhours to C/YP at end of life.

    You are required to lead a team of practitioners and support other staffacross teams in the wider Diana Service, therefore excellent communication andstrong leadership skills are essential. The post is positioned within theDiana leadership team and therefore will have additional, specified leadershipresponsibilities across the service and FYPC/LD.

    Main duties of the job

  • Clinically lead the Continuing Care Team within the Diana Service, in line withpolicy and procedure to enable the safe management of specific health needs andappropriate delegation of aspects of care, using necessary documentation forLeicestershire Partnership Trust.
  • Support the Continuing Care Team deliveringpackages of care in the community in a safe and effective way, workingalongside children/young people and their families, acute hospitals,commissioners, and the wider MDT.
  • Support other members of the Diana Leadershipteam in the delivery of effective leadership and communication, attendance at meetings providing expertise and information.
  • Alongside the Diana Service Clinical Leadsdevelop the annual training programme for the workforce across the Diana team.Ensure that staff are supported to achieve service expectations for clinicalcompetence.
  • Develop the service to ensure thehealth needs of children in the community are met highlighting any gaps tomanagement and commissioners.
  • Ensure and support the current nursing workforceto meet current service demand in relation to clinical competence.
  • Proactively contribute to the longer-term workforce development to ensure that the service can meet future clinical demands.
  • To participate in the on-call service providedto staff and end of life children and young people within LLR.
  • We are currently based at Bridge Park Plaza, Thurmaston. We will be moving to County Hall, Glenfield Hospital site from Spring 2024.
